Tool for Incorporating Human Factors During Process Hazard Analysis (PHA) Reviews of Plant Designs provides additional human factors guidance and tools in the petroleum and petrochemical industry. Use of the Tool is intended to help eliminate incidents by identifying and mitigating human factors issues during process hazard analysis. It will also assist facilities to meet the requirements of OSHA’s Process Safety Management (PSM) and EPA’s Risk Management Plan (RMP) rules for addressing human factors during hazard reviews.
